Position Descriptions

SASH Coordinator
The Support And Services at Home (SASH) Coordinator is to build trusting relationships with participants in SASH in order to develop a thorough knowledge of the each SASH participant’s strengths and challenges as they pertain to remaining safely in his or her home.  The SASH Coordinator convenes the on-site SASH team on a regular basis to coordinate care and services to meet the needs of SASH participants.  The goal of the SASH model is to enhance the coordination of person-centered health and wellness services in order to assist residents to remain in their homes as their health and functional needs change.  SASH Coordinator is also responsible for oversight of the day to day operations at the properties, including; reviewing property’s annual budget(s) and oversight of expenses, and compliance with all regulatory requirements.

Qualifications:  Must possess a Bachelor’s Degree in Social Work or equivalent combination of background and experience.  Previous experience coordinating services specifically for the senior or adult with disability client population is preferred.  Knowledge of area resources and programs available to seniors and adults with disabilities, including transportation, health services, and recreational activities is essential.  Demonstrated excellence in verbal and written communication is required. Must have knowledge of and an appreciation for the heritage, values, and wisdom of each resident and a commitment to the philosophy of a person’s choice to age at home.  Must possess a valid driver’s license.  A working knowledge of MSWord and Publisher is highly desirable.

 

SASH Wellness Nurse
The Support And Services at Home (SASH) Wellness Nurse is responsible for the provision and/or oversight of wellness care and preventative services for residents at assigned communities.  He/she is responsible for ensuring the well-being of the residents and ensures direct care services are of a high quality.  He/She supports a philosophy of aging in place consistent with the mission of CSC. As part of the SASH team, assists in the development and implementation of a healthy aging plan for each resident, conducts annual health and wellness assessments and updates as needed coordinates, schedules, and provides foot care clinics, provides nursing services such as:  blood pressure, heart rate, weight monitoring, medication review, overall health status, information/education training, and answers questions the resident may have.

Qualifications:  Must be currently licensed as a Registered Nurse in the State of Vermont.  Must possess at least two years of experience in a clinical setting, preferably within an assisted living residential care or similar setting committed to an aging in place philosophy.  He/she must have knowledge of standard record keeping procedures (i.e. progress notes, HIPPA guidelines).  Must be able to work independently, and possess strong organizational and time management skills and excellent verbal and written communication skills.
